The ratio is simple: waist circumference divided by height, in the same units. Repeat under the same conditions and combine it with blood pressure, glucose, lipids and fitness.
Waist ÷ height, using centimeters for both. Example: 88 ÷ 176 = 0.50.
Measure midway between the lowest rib and top of hip after a normal exhale; do not pull the tape tight.
Below 0.50: generally lower central risk; 0.50–0.59: review risk factors; 0.60+: stronger reason for clinical assessment.

Large reviews and cohort studies show that waist-to-height ratio predicts cardiometabolic and mortality risk at least as well as BMI in many populations.

What we cannot claim
The 0.5 threshold is a practical public-health heuristic. It does not identify disease, and the best cutoff can vary across populations and outcomes.
