HOMA-IR is a model of a fasting steady state
The original model uses basal glucose and insulin as a simple approximation of insulin resistance. HOMA2 is a different nonlinear computer model and its output should not be mixed with HOMA1.

Important limitations
What the number can—and cannot—tell you
What HOMA-IR number means insulin resistance?+
There is no universal cutoff. Published thresholds change with population, assay, age, metabolic status and study purpose. Avoid treating an internet band as a diagnosis.
Can HOMA-IR diagnose prediabetes or diabetes?+
No. Diagnostic criteria use laboratory glucose or HbA1c testing under defined conditions, usually with confirmation when hyperglycemia is not unequivocal.
Why might results change between laboratories?+
Insulin assays can differ materially. Repeat comparisons are cleaner with the same laboratory, similar fasting conditions and similar timing.
